CMS needs Primary Care practices to enroll

The CMS Innovation Center has a growing portfolio testing various payment and service delivery models that aim to achieve better care for patients, better health for our communities, and lower costs through improvement for our health care systems. Learn more when you select your state under “Where Innovation is Happening” to see where Model Partners, such as the Primary Care First Model Options  summary and request for applications are now available. The alternate payment model is oriented around five comprehensive primary care functions: access and continuity, care management, comprehensiveness and coordination, patient and caregiver engagement, and planned care and population health, the fact sheet says.
