Talking about the “what if’s” is never an easy thing to do. Especially when we are talking about our own
health and mortality. However, this is something that is so important in this age of scattered families
and managed care. The time for a health care proxy or advanced directive is not during a crisis rather when you are healthy and clear minded. To properly prepare, before a health crisis, requires decisions about how you want to be cared for and a great gift to your family.
Talking about your wishes now will help them to know what your expectations
and desires are. Person-centered care identifies your wishes so meaningful dialogue with you loved ones
can speak to best practice and care possibilities.
Here’s a few things to consider:
- A Healthcare Proxy- someone whom you authorize to make medical decisions for you if you
can’t do so for yourself - A Durable Power of Attorney (POA)- someone that will handle legal and financial issues for you if
you cannot do so for yourself and/or become incapacitated. - Advanced Directives- your choice, what care you want as well as what you don’t want
